AI for prevention and earlier detection

Proactive care requires a useful response to an earlier signal. Finding more abnormalities is only one part of evaluating an AI screening or risk prediction program.

Follow the whole pathway

Describe the pathway from risk estimate to clinical review, confirmatory testing, a care decision and an outcome. Assign responsibility at each transition. A signal that nobody can act on may create anxiety or work without improving care. Screening people without symptoms also raises different questions from evaluating someone who is already unwell.

Earlier does not always mean better

The National Cancer Institute explains how lead time and overdiagnosis can make screening statistics misleading. Earlier diagnosis can increase measured survival from diagnosis without extending life. Detection can also uncover disease that would never have caused symptoms. An AI screening study therefore needs interpretation beyond the number of findings. National Cancer Institute: cancer screening overview.

Questions for a prevention program

The following is an editorial planning checklist, not a screening recommendation.

  • Which population is the tool intended for, and who should not use it?
  • What action follows a positive result, and is that service available?
  • How many false alarms, missed cases and unnecessary procedures occur?
  • Are benefits measured through patient outcomes or only through detection?
  • Who may be excluded by cost, language, device access or poor data quality?
  • How will the team compare benefits, burdens and costs with usual care?

For patients and caregivers

Discuss screening decisions with a qualified clinician who can consider age, history, symptoms and preferences. An AI risk score cannot establish an individualized testing schedule on its own. If a service advertises earlier detection, ask what happens after the result and what evidence shows that the entire care pathway helps people like you.
